Overview
Provides general medical care and treatment to patients in medical facilities such as clinics, health centers, or public health agencies under the direction of a Physician by performing the following duties.
Responsibilities- Perform physical examinations and preventive health measures following Physician‑approved guidelines.
- Order, interpret, and evaluate diagnostic tests to identify and assess patient conditions.
- Record findings, formulate care plans, and develop prognoses based on patient status.
- Discuss cases with Physicians and other health professionals to prepare comprehensive care plans.
- Submit patient care plans and goals for periodic review by a Physician.
- Prescribe or recommend drugs and non‑pharmacologic treatments such as physical therapy, inhalation therapy, or other therapeutic procedures.
- Refer patients to specialists or appropriate health resources.
- Travel to perform home visits and occasionally travel for patient care duties.
- Regularly stand, walk, sit, handle, reach, speak, and hear.
- Occasionally climb, balance, taste, and smell.
- Require close vision, depth perception, and ability to adjust focus frequently.
- Require distance vision, color vision, and peripheral vision occasionally.
- Occasionally exert up to 50 pounds of force and frequently up to 25 pounds of force.

- Graduate of an accredited nursing program.
- Graduate of an accredited master’s of nursing program.
- Meet the requirements determined by the State of Wisconsin or Illinois to perform duties defined as those of a Nurse Practitioner.
- Hold a valid RN license in the state of practice.
- Hold a valid APNP license in the state of practice.
- For Home Health and Post Acute Care: maintain valid licensing and adhere to fleet and driver safety policies.
- Excellent interpersonal and communication skills.
- Analytical and interpretive skills.
- Indirectly supervises other palliative care nursing or support staff.
