Role OverviewA registered nurse (RN) is a professional nurse responsible for leadership, direction and clinical supervision of the health care team, including licensed practical nurses, medication assistants and certified nursing assistants. The RN provides nursing care and coordinates care for residents in collaboration with an interdisciplinary team and supervises direct patient care staff.
Responsibilities- Administers medications and performs treatments:
- Administers routine medications via oral, subQ, IM, nasal, eye, ear, vaginal, rectal, inhaler and nebulizer routes; observes effects and reports unusual reactions.
- Administers IV therapy, piggyback medications, heparin and saline flushes; completes blood draws.
- Administers PRN medications.
- Performs treatments such as dressing changes, tracheostomy care, skin tear protocol, catheter & colostomy care, G/J tube care and feeding, peritoneal dialysis, oxygen administration, suctioning, nebulizer use.
- Provides emergency care including CPR.
- Performs EKGs and bladder scans.
- Collects and records data (20% of time):
- Reports changes of condition to supervisors, nurse manager and physician; ensures continuity of care through shift reports.
- Counsels and provides health teaching to residents and families.
- Interacts with residents, families and agency representatives on an ongoing basis.
- Participates in planning and implementation of residents’ plans of care.
- Documents medication administration, treatments, progress notes, monthly summaries, comprehensive summaries, quarterly assessments and restorative nursing notes.
- Reviews electronic health records and co-signs incident reports, orders and LPN information as necessary.
- Functions as team leader on the unit and during code situations (25% of time):
- Ensures a safe environment for residents, staff, families and visitors.
- Provides guidance and supervision to licensed practical nurses, medication assistants and certified nursing assistants.
- Participates in quality assurance and other committees as assigned.
- Attends required in‑service programs, both in person and online.
- Monitors and assists with resident dining (10% of time):
- Ensures care needs of high‑risk residents are met.
- Performs other duties as assigned (5% of time).
- Knowledge of medical, geriatric and psychiatric nursing theory.
- Knowledge of medication administration including psychoactive medications.
- Knowledge of MDS, RAPs and utilization guidelines; familiarity with state and federal nursing home survey processes.
- Knowledge of client population needs and treatment sources.
- Effective oral and written communication.
- Ability to present information and respond to inquiries from managers, staff, residents, families, professionals and the public.
- Team‑working and relationship‑building skills with physicians, staff and management.
- Ability to recognize and respond to urgent/emergent situations.
- Multitasking efficiency.
- Reviewing and assuring clinical work of others meets professional standards.
- Demonstrates responsibility, accountability and commitment to professional growth.
- Maintains professional appearance and conduct.
- Manages typical job stress.
- Performs related work as assigned.
- Registered Nurse with current Wisconsin licensure in good standing.
- CPR certification or documented attendance at CPR updates.
- Long‑term care experience preferred.
- May be required to stay past the normal shift until relieved to meet staffing requirements.
- Ability to sit, stand, bend and stoop for periods of time.
- Ability to lift up to 50 pounds occasionally.
- Work in a stressful environment and respond to emergency/crisis situations.
- May physically push/pull medication carts.
- Exposure to noise, blood and/or body fluids.
- Use of personal protective equipment (gloves, gowns, goggles, protective shield) as needed.
- Enter accurate information into charts and reports.
- Potential exposure to hostile or emotionally upset residents, families, personnel, visitors.
- Potential exposure to falls, burns, odors, infectious waste, diseases, blood and body fluids.
- Work inside a skilled nursing home facility; campus is smoke‑free.
- CPR/AED certification required.
