Position

LSS of WI & Upper MI is currently seeking a Housing & Urban Development (HUD) Service Coordinator to serve our tenants in Janesville, WI. The role is based in Janesville and will require travel around the area supporting the needs of residents in the community.

Our program is designed to promote and support self-sufficiency of individuals residing in properties, facilitating a variety of support services within the residential community. Our role as Service Coordinators is to connect residents with services in the community with the goal of reducing early admission to nursing homes or assisted living programs, promote independence, and help our clients stay independent for as long as possible.

This is a full-time opportunity at 40 hours per week. This position will support residents at Riverview Heights, located at 930 N. Washington St., Janesville, WI 53548.

Essential Duties and Responsibilities
  • Under the direction of the Program Supervisor, provides information and referrals to community connections to tenants following guidelines, as defined in the HUD Housing Handbook Section 202, who need supportive community services to maintain self-sufficiency.
  • Provide general informal case management, including intake, and referral services to tenants needing such assistance when case management is not available.
  • Assessment of health, psychological and social needs. Development of an individually tailored case plan for services and periodic reassessment of the tenant's situation.
  • Establishes linkages with all agencies and service providers in the community; develops best-value service options to ensure individualized, flexible, and creative services for tenants.
  • Set up a directory of providers for use by staff and tenants.
  • Refer and link tenants to service providers in the general community such as case management, personal assistance, homemaker, meals on wheels, transportation, counseling, visiting nurse, preventative health screening, wellness, and legal advocacy.
  • Educates tenants on service availability and procedures, providing advocacy as appropriate.
  • May develop case plans in coordination with community assessment services.
  • Monitors ongoing provision of services and keeps case management and providers informed of progress.
  • May set up volunteer support programs with community service organizations.
  • Help other tenants build informal support networks with residents, family and friends.
  • May provide training to tenants regarding tenancy obligations or coordinate such training.
  • May educate staff on aging in place and service coordination to better assist tenants.
  • Presents workshops or informational sessions to tenants on topics of interest.
  • Demonstrates ability to work with tenants, the agency, and the community in a positive manner and to work independently.

Education and/or Experience

Bachelor s degree in social work, sociology, psychology or related field or an equivalent combination of education and/or experience is required with at least 3 years of related experience, including experience working with older adults. Working knowledge of supportive services and other resources for older adults and/or persons with disabilities is strongly preferred.

Certificates, Licenses, Registrations

The incumbent must have a valid driver s license and reliable transportation; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.
