Position Summary
The Children's Long Term Support (CLTS) Program Specialist is responsible for completing intake and calculating eligibility for children referred to the CLTS program, conduct outreach and assessments for eligible/potentially eligible children, and enter children into the Eligibility and Enrollment Streamlining System. The position collaborates with multiple systems, internal partners, and community resources.
Key Responsibilities- Conduct functional screens and assessments for children (40%)
- Complete required case management documentation (20%)
- Follow up with children in crisis and collaborate with partners (15%)
- Answer phones and emails daily (15%)
- Conduct quality assurance on functional screens (5%)
- Other duties as assigned (5%)
- Conduct functional screens per waiver manual guidelines
- Assess internal cases for children in out-of-home placements
- Connecting case management duties to provide services to children on the wait list and utilize other supports and resources while they are waiting
- Monitor and evaluate health and safety needs of children and provide supports that will ameliorate those needs
- Respond to client crisis needs to support the individual and families
- Assess for suicidal/homicidal risk and make referrals as necessary
- Discharge planning
- Report critical incidents
- Provide advocacy and ongoing evaluation of service providers
- Provide guidance to families on the continuum of care, transition process and guardianship process for transition age youth
- Document contacts with families and collateral partners
- Document critical incidents and restrictive measures
- Document in Avatar all client related activity
- Completion of TPA authorizations
- Gathering and sharing information, supervision, make recommendations about variance requests, assist in managing waitlists
- Answer phones/emails, return calls timely
- Conduct quality assurance on functional screens and participate in review panel for not functional eligible CLTS functional screens
- Provide information to other case managers about screen questions
- Bachelor's Degree in counseling, social work, psychology, or human services related field
- Social Work Certification Preferred
- Minimum of 2 years of CLTS experience, in good standing with the agency
- Certified in Children's Long Term Care Functional Screen, experience conducting Children's Functional Screens
- Current unrestricted driver's license and auto insurance
- Knowledge of Home and Community Based Waiver Manual
- Knowledge of county-wide resources for children, youth, and families at risk
- Knowledge of evidence based practices with children, youth, and families
- Knowledge of variety of disabilities, court systems and legal processes
- Knowledge and ability to document and manage data in Microsoft Word, Excel, and Avatar
- Understanding of cultural competence and working with diverse populations
- Ability to communicate effectively orally and in writing
- Ability to work and make decisions independently
- Ability to engage children, youth, and families with risk factors for a mental health crisis, out-of-home placement, and court system involvement
- Ability to work in fast paced, changing environment; prioritize duties, meet deadlines
- Ability to travel throughout the county on a daily basis; flexible schedule
- Ability to have a flexible work schedule; valid driver's license and insurance; able to work evenings when needed
- Knowledge of equipment use: computer, copy machine, fax machine; type notes daily using Microsoft Word, Excel, Avatar
- Working conditions:
multiple locations; exposure to potentially abusive clients - Physical elements: stand, sit, walk; active with children and youth
Ability to work in multiple locations including the office, client's home, and a variety of community locations. Valid Driver's License and insurance. Available to work a flexible schedule including evenings when needed to meet client needs. Potential exposure to physically or verbally abusive clients.
