Albert Road Surgery are run by South Tyneside Health Collaboration - a federation of South Tyneside GP Practices.
Theremay be a need to work across our other sites based in South Tyneside.
The job is STHC Payscale E, which equates to £38,032 - £45,965 per annum

- Immunisations Inc. children and travel
- Health Screening
- Infection control
- Wound care
- QOF management

We are a well-established surgery and our team consists of:

- Team of 7 reception/admin staff
- Practice Manager
Tobe responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.
Workingas a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
The Practice Nurse will be responsible for a number of clinical areas such ashealth promotion, chronic disease management, and mentoring other members ofstaff. Actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
Albert Road Surgery will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation toprovide general advice and support on a day-to-day basis.
Person Specification Qualifications- Registered General Nurse Full NMC registration experience in chronic disease management.
- Mentor or teaching qualification Post graduate diploma or degree (Chronic Disease Management) Family Planning Qualification prescribing
- Experience of working in a GP Practice
- Primary Care Experience of chronic disease management
- Independent/ supplementary prescriber
- Interest in women health/ contraception/HRT with accredited qualifications
