Description:

Summary — Responsible for the overall comprehensive care provided to the patients in the CHRISTUS SETX Health System. The NP will coordinate care with the Medical Director, conduct patient education, ensure accurate and timely documentation in accordance with regulating agencies and organizational policy, participate in community agencies and events, and work with the team to coordinate patient management and individualized plans of care.

• Take medical histories and perform physical exams.
• Order and interpret laboratory studies and provide treatment to patients with chronic diseases.
• Coordinate patient management with the Medical Director and other team members; ensure follow-up calls and identify additional visits or interventions as needed.
• Manage and adjust medications and educate patients; collaborate with the RN and Medical Director to optimize patient care.
• Participate in community events and marketing opportunities involving CHRISTUS SETX Health System.

• Current Licensure through the Texas Board of Nursing OR Licensed Physician Assistant by the Texas State Medical Board or the ability to obtain prior to employment.
• Valid DEA license or the ability to obtain one prior to employment.
• BLS obtained by the American Heart Association.
