Case Manager – Jeannette, PA
Location: Jeannette, PA – Agency-owned home with dedicated foster parent for sibling groups.
OverviewAdelphoi Village is seeking a compassionate and qualified professional to work with siblings and a foster family in a foster care setting.

Job Responsibilities- Provide ongoing support and deliver case‑management services for up to six youths in residence; may assign additional youth cases in other foster homes as needed.
- Develop individualized service plans to inspire emotional, intellectual, and social growth.
- Build relationships while setting boundaries for the foster parent in residence, the children under care, the birth family, children and youth services, and court‑related authorities.
- Respond to crisis situations and provide immediate assistance when needed.
- Maintain accurate records in Adelphoi’s Electronic Health Record system, documenting communications, reports, Individual Service Plans, case‑work session notes, and other required documents.
- Sustain regular communication with all stakeholders.
- Assess foster family and environment continuously to ensure physical and psychological safety; provide trainings; maintain all required paperwork for foster family licensure and re‑certification annually.
- Assist with family visitation when needed.
- Manage a normal Monday‑through‑Friday schedule with flexibility for school and youth needs; may require evenings and weekends occasionally.

- 1–3 years of youth/family counseling or case management
- Bachelor’s degree in psychology, social work, counseling, or related behavioral disciplines
- Strong one-to-one and small group communication skills
- Knowledge of the foster care system and related regulations preferred
- Valid driver’s license and car insurance
- FBI fingerprinting, Act 33/34 child‑abuse clearance, PA criminal background check (agency assistance provided upon hire offer)
