• Establish days and hours of practice.
• Control patient schedule and set fee schedule.
• Keep 100% of exam/professional fees.
• Opt into equipment/instrumentation and opt‑tech support if permitted.
• Provide eye and vision care and promote well‑being.

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Doctor of Osteopathic Medicine (DO) or Medical Doctor (MD) from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W‑9.
