We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN/ LVN) to join our healthcare team. As a LPN/LVN you will be responsible for providing high-quality, patient-centered care to residents and ensuring a safe and supportive environment. You will play a vital role in enhancing the quality of life for our patients by delivering excellent clinical care, managing patient interactions, and collaborating with other healthcare professionals.

Essential Duties- Administer medications, treatments, and wound care as prescribed by physicians.
- Monitor and assess patient conditions and promptly report changes.
- Assist in patient admissions, discharges, and transfers.
- Provide compassionate care, ensuring patients' comfort and dignity.
- Maintain accurate patient records and documentation in accordance with facility policies.
- Coordinate with doctors, CNAs, and other healthcare team members to deliver comprehensive care.
- Educate patients and families on health management and discharge instructions.
- Current LPN/LVN license in Indiana
- Strong clinical and patient management skills
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and handle challenging situations with grace
- Basic Life Support (BLS) certification required;
Advanced Cardiac Life Support (ACLS) preferred
