- Deliver home-based care to patients recovering from illnesses, injuries, and chronic diseases, often post-hospital discharge, under RN supervision.
- Collaborate with a dedicated team of physicians, RNs, CNAs, and aides to ensure quality patient care.
- Monitor patient vitals, conduct assessments, document progress, and communicate changes to your RN supervisor.
- Assist RNs in implementing patient care plans and teaching self-care techniques.
- Educate families on care plans, medication, and home safety.
- Work with an interdisciplinary team to coordinate patient care and plan for discharge.

- Your work-life balance is just as important to us as it is to you!
- Active, unrestricted LPN license in the State of Michigan.
- At least one (1) year of clinical experience as an LPN.
- Current CPR Certification.
- Reliable transportation, valid state driver's license, and current car insurance.
