* Arrange and coordinate service referrals for the client/family as needed; provide ongoing monitoring of the activities implemented to carry out the plan of care. Remove access barriers by referring clients/families to public benefits programs as eligible.
* Provide regular contact with enrolled clients and document all contacts and service coordination activities in client records as required by program guidelines.
* Assist youth to secure needed medical follow up/services and monitor kept appointments.
* Maintain current knowledge of and incorporate evidence-based interventions for youth, and routinely interact and network with community providers, clinicians, and others involved in youth service delivery. Advocate for improved services for youth.
* Work in coordination with other SBHC staff and Jennings High School staff to develop and implement a range of youth-centered programming that will address primary issues impacting the lives of youth clients. Co-facilitate needed programs and/or work with community partners to develop/provide needed services.
* Partner with SBHC staff to work from a youth empowerment/developmentally appropriate framework to assure that youth are active partners in all aspects of the services received at the center.
* Perform indirect, non-clinical tasks related to the job, including compilation of data, participation in required training, and attendance at staff meetings and community meetings as assigned to meet reporting requirements.
* Work collaboratively within the multidisciplinary SBHC team, Jennings High School faculty and administration and community partners.
* Act as point person for the coordination of behavioral health services and work in partnership with the Project ARK/The SPOT Director, and Medical Director to implement the project; participate in leadership activities and meetings.
* Attend meetings as scheduled to coordinate, advocate and plan for youth services.
* Perform indirect, non-clinical tasks related to the job, including regular attendance at staff meetings and timely compilation of client data upon request.
* Basic Life Support certification must be obtained within one month of hire date
* Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
