Position Summary: Position will provide youth-centered case management/referral services for high school age youth as part of a school-based health center (SBHC) located at Jennings High School. The position will coordinate with other behavioral health/health staff members of the SBHC multidisciplinary team and closely with school faculty and leadership to implement services within the school setting.

This team will create care plans that include identification of social, mental health, health prevention, and educational/vocational needs. The position will be dedicated to and experienced in working with diverse youth/families of all races, sexual orientations, and identities. The role will utilize assessments and screening to identify areas of concern and provide community resource referral, crisis intervention, supportive counseling, and coordination of necessary resources.

Will abide by the same strict standard of patient confidentiality that applies to all WashU staff.

Job Description Primary

Duties & Responsibilities
  • Conduct a culturally sensitive comprehensive psychosocial assessment with the client and family/support network as appropriate, to develop a plan of care to meet identified needs.
  • Arrange and coordinate service referrals for the client/family; provide ongoing monitoring of activities implemented to carry out the plan of care and remove access barriers by referring clients to public benefits programs as eligible.
  • Maintain regular contact with enrolled clients and document all contacts and service coordination activities in client records as required by program guidelines.
  • Assist youth to secure needed medical follow‑up/services and monitor attended appointments.
  • Stay current with and incorporate evidence‑based interventions for youth; routinely interact and network with community providers, clinicians, and others involved in youth service delivery; advocate for improved services.
  • Collaborate with SBHC staff and Jennings High School staff to develop and implement youth‑centered programming that addresses primary issues impacting youth clients; co‑facilitate needed programs or partner with community providers.
  • Partner with SBHC staff to promote a youth‑empowerment/developmentally appropriate framework so youth are active partners in all aspects of the services received.
  • Perform indirect, non‑clinical tasks related to the job, including compilation of data, participation in required training, and attendance at staff and community meetings as assigned.
Additional Duties
  • Work collaboratively within the multidisciplinary SBHC team, Jennings High School faculty and administration, and community partners.
  • Act as point person for coordination of behavioral health services and partner with the Project ARK/The SPOT Director and Medical Director to implement the project and participate in leadership activities and meetings.
  • Attend meetings as scheduled to coordinate, advocate, and plan for youth services.
  • Maintain timely compilation of client data and regular attendance at staff meetings.

Required Qualifications

Education: Master’s degree or a combination of education and experience may substitute for minimum education.

Certifications / Professional Licenses: Basic Life Support – American Heart Association, Basic Life Support – American Red Cross.

Work Experience: No specific work experience required for this position.

Driver's License: Not required.
