• Manage a dedicated patient panel, delivering ongoing in-home primary care.
• Care for members with chronic illnesses and advanced medical needs.
• Participate in a shared on-call rotation to ensure continuous support.

• Located in or near Essex, New Jersey and able to travel locally for home visits.
• Active, unencumbered NJ Registered Nurse license.
• Certified Advanced Practice Nurse Practitioner (APN).
• Master’s degree or post-master’s certificate in advanced clinical practice.
• Minimum 1 year of NP experience in a clinical setting.
• Active DEA and CDS registration.
• Current Basic Life Support (BLS) certification.
• Experience in home care or house call settings (preferred).

A reasonable estimate of the base salary range for this role is $140,000 to $165,000.
