Catholic Charities of the Archdiocese of Newark is one of New Jersey’s largest non-profit social service agencies with programs and facilities serving Bergen, Essex, Hudson, and Union counties.
We are currently seeking a Full Time Case Manager for our St. Lucy’s Emergency Homeless Shelter
, located in Jersey City, NJ
. St. Lucy's is an 80 bed emergency shelter in Jersey City that ensures safety and builds stability in the lives of homeless men and women.
- The Case Manager is responsible for the overall delivery of social services, intake, assessments, admissions, discharges, development & implementation of Service Plans, homeless prevention services and related documentation and progress reports.
- Develop and maintain compliance with HUD, County and DCA essential services standards and related policy and procedures.
- Maintain referrals and communication with County Coordinated Assessment Agency.
- In partnership with the family, develop a Housing & Service Plan and provide assistance and linkages in obtaining needed services and achieving clients’ goals.
- Bachelor’s Degree required (Bachelor’s degree in social work or related field preferred).
- 2-3 years experience working with the homeless population or related experience required.
