Crescent Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Augusta. This full-time role delivers hands-on end-of-life care, coordinates services, and guides patients and families through the hospice journey with a focus on comfort and dignity.
The position requires an active RN license, at least one year of medical-surgical/acute care experience (hospice is a plus), and proficiency with EMR systems. A valid driver’s license and CPR may be required.
