Home Health Nurse Case Manager — Care Coordinator

United Health Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinating care and supporting discharge and transition planning. Under supervision of an RN or MD, you will ensure appropriate care levels and provide education, coaching, and decision support.
