Director of Hospice Services
Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care, and grief support for adults and children.
The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non‑clinical staff are based out of our Joliet administrative offices.
We are currently seeking a full‑time Director of Hospice Services to assist with the overall direction of hospice clinical services. The role will be responsible for:
- Coordinating and overseeing all direct and indirect patient services provided by clinical organization personnel.
- Providing guidance and counseling to direct reports, including Team Leaders, to continually improve all aspects of hospice care services.
- Assisting Team Leaders, when applicable, in managing clinical teams and planning.
- Assisting with assessment, planning, implementation, and evaluation of patient and family/caregiver care for all clinical personnel as indicated.
- Monitoring, assessing, and implementing changes to improve Consumer Assessment of Healthcare Providers and Systems Survey (CAHPS) scores.
- Managing all staffing levels (hire, evaluation, and termination) of organization personnel.
- Conducting staff evaluations annually or more frequently if indicated.
- Assisting with on‑call services as scheduled.
Qualified candidates will be an RN in Illinois with a Bachelor's degree; a Master’s degree is preferred. Candidates should also have a minimum of three years of management experience in hospice or a related healthcare field.
