Job SummaryUnder the direct supervision of the Primary Care Manager, the applicant will provide general nursing care and leadership while working collaboratively with physicians and other multidisciplinary team members. Instructs individuals and families in health promotion and disease prevention; provides instruction in general health care and treatment to assigned clients in branches and clinics, working in concert with family physicians as necessary.

The candidate will be expected to work in the clinic's family, OB, Infectious Disease, and BH areas and travel to satellite offices. Schedule calls for Monday through Thursday, 9:00 am-5:00 pm, and Friday, 8:30 am-4:30 pm.

Minimum QualificationsHigh School Diploma or equivalent. Licensed Practical Nurse (LPN) certification by the State of Illinois or a reciprocal State is required.

RequirementsVerification of U.S. citizenship or employment eligibility. Must possess a valid Illinois Driver's License with minimum auto liability insurance.

Salary Range$51,108 (Starting Salary (Grade 12, Step 13) to $72,212(Top of the range (Grade 12, Step 27)
