MDS and Restorative Nurse RN or LPN

At Joliet Living and Rehab Center LLC, we are seeking a dedicated and compassionate MDS and Restorative Nurse RN or LPN to join our team of healthcare professionals. As a key member of our nursing staff, you will play a vital role in providing exceptional patient care and ensuring the highest level of satisfaction for our residents.

• Conduct comprehensive resident assessments, including Minimum Data Set (MDS) assessments, to identify care needs and develop individualized care plans.

• Implement restorative nursing programs designed to improve residents' functional abilities and overall well-being.

• Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals to ensure comprehensive care.

• Develop and maintain accurate and detailed records of resident care, assessments, and progress.

• Provide emotional support and comfort to residents and families, promoting a positive and respectful environment.

• Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Illinois.

• Ability to work varying shifts, including evenings, weekends, and holidays.

• Strong communication and interpersonal skills, with the ability to effectively interact with residents, families, and healthcare professionals.
