Community Support Specialist I – AUNT MARTHAS HEALTH AND WELLNESS, Inc., Joliet, IL, United States

SUMMARY

Aunt Martha's Health and Wellness is committed to supporting community well‑being, ensuring equitable access, and delivering exceptional care. The Community Support Specialist I acts as a stabilizer, providing assessment, counseling, service linkage, individual, family, or group therapy, school advocacy, life skills training, and educational and therapeutic group activities to 10‑17‑year‑old youth at risk of running away, parental neglect, or involvement in the juvenile justice system.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Daily

Job Duties:

  • Respond to all emails, texts, and phone calls from the Program Coordinator within a 15‑minute time frame.
  • Ensure the Outlook calendar is updated with daily tasks and travel in real time.
  • Maintain an active caseload of 20‑25 participants with a maximum of 15 higher‑risk clients (trauma, suicide risk, mental health, etc.) as assigned.
  • Facilitate groups for discretionary clients within designated LANs.
  • Ensure all required crisis intervention and intake documentation is complete and accurate.
  • Maintain active CCBYS client files and keep documents no more than two weeks overdue.
  • Update and maintain the designated LAN client, MRAI/referral, outreach, and quarterly success stories spreadsheet.
  • Monitor and supervise clients to ensure safety and well‑being during programming hours and transport.

Other Duties:

  • Assist the Community Liaison with community relationships, monthly client baskets, events, and staff meetings.
  • Help plan and execute CCBYS staff events, lunch & learn presentations, and annual outreach meetings.
  • Obtain Letters of Support and Memorandums of Understanding from community partners annually.
  • Participate in supervision, team meetings, and trainings to address client issues and documentation updates.
  • Provide required documentation (mileage, timesheets, CQIs, surveys) as indicated by agency protocols.
  • Submit all necessary paperwork, including vouchers, progress notes, incident reports, and timesheets, in a timely manner.
  • Perform other duties as assigned by Program Coordinator, Manager, or Director.
QUALIFICATIONS

EDUCATION AND OR EXPERIENCE

Bachelor's degree (BA/BS/BSW) required. Previous experience with at‑risk youth preferred.
