Discharge Planner - Bsw

JOB REQUIREMENTS

• Education
  • Must be licensed as a Registered Nurse, a Social Worker, or a Masters Social Worker, or be a graduate of a Bachelor of Nursing program or a Bachelors or Masters in Social Work program or other related field.
• Experience
  • One year hospital experience preferred. Demonstr knowledge and skills necessary to provide care to and communicate with primarily the geriatric population and adult population. Demonstrates knowledge of the principles of growth and development over the life span. Able to assess data reflecting the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs.
• Physical
  • Involves being inside and outside. Exposure to temperatures 32 degrees and less and 100 degrees or more, fumes, dust, and noise. Hearing within normal range. Frequent sitting, stooping, standing, walking, bending, pushing/pulling up to 250 pounds, carrying up to 35 pounds, and lifting up to 100 pounds. Must have reliable transportation and current drivers license valid in the state of Arkansas and automobile liability insurance.

The discharge planner will coordinate services needed upon discharge, including sending referrals to appropriate facilities such as nursing homes and residential care facilities. Discharge planner will also arrange transportation when appropriate and will coordinate outpatient mental health services.
