• The discharge planner will coordinate services needed upon discharge, including sending referrals to appropriate facilities such as nursing homes and residential care facilities.
• Discharge planner will also arrange transportation when appropriate and will coordinate outpatient mental health services.

• Must be licensed as a Registered Nurse, a Social Worker, or a Masters Social Worker, or be a graduate of a Bachelor of Nursing program or a Bachelor or Masters in Social Work program or other related field.
• One year hospital experience preferred.
• Demonstrated knowledge and skills necessary to provide care to and communicate with primarily the geriatric population and adult population.
• Demonstrates knowledge of the principles of growth and development over the life span.
• Able to assess data reflecting the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age‑specific needs.

• Involves being inside and outside; exposure to temperatures 32°F and below and 100°F or above, fumes, dust, and noise.
• Hearing within normal range.
• Frequent sitting, stooping, standing, walking, bending, pushing/pulling up to 250 pounds, carrying up to 35 pounds, and lifting up to 100 pounds.
• Must have reliable transportation and current driver's license valid in the state of Arkansas and automobile liability insurance.
