Community Health Worker (CHW) – Joplin, MO
What You’ll DoProvide care coordination and social needs assessment for patients and families, develop strategies for care coordination, and facilitate connections to appropriate resources to meet health and social needs.
Requirements- High school diploma or equivalent.
- 1–3 years of experience in a community-based setting.
- Community Health Worker – State of Missouri certification within 9 months of hire.
- Associate’s degree.
- Bachelor’s degree.
