This is not a remote position. All candidates will be required to be either on the island of Maui, or willing to relocate to Maui.

Imua Family Services is looking for Care Coordinators/Social Workers to become a part of our team. This position will provide care coordination and management for families of children 0-8 with developmental delays. Imua Family Services is a non-profit serving Maui, Molokai, and Lanai since 1947. Our mission is to empower children and their families to reach their full potential.

Summary:
This position is responsible for the provision of family-centered culturally competent services for children and their families, birth to eight with typical development and developmental delays. The individual uses a primary service provider and coaching approach to instruct families and their children in activities designed to promote social, physical and intellectual growth needed for child development in their natural environments. The individual compiles the necessary information and data for programming and to create accurate reports and billing for various funding sources.

Essential duties and responsibilities also include providing information to other professionals and the community as well as conducting other clerical work and administrative duties as required.

Minimum Qualifications

• Bachelor's Degree in related field from an accredited college/university required.
• Licensed Social Worker/Ability to become LSW or LCSW preferred.
• Pediatric experience with birth to eight populations is preferred.

Salary starting at $45,000/annually, Commensurate with Experience

This position provides interim and on-going care coordination services to families receiving services from Imua Family Services. The individual compiles information and the data necessary for programming and for accurate reports and billing to the Department of Health, Medicare medical insurance, and various funding sources. Essential duties and responsibilities also include providing information to other professionals and the community as well as conducting other clerical work and administrative duties as required.
