Social Services Assistant – Hale Makua Health Services
Hale Makua is a Maui‑based nonprofit that improves the well‑being of those in its care and is devoted to helping Maui's elders live their best lives. Join our healthcare social services team in a long‑term care and skilled nursing facility on Maui to support our kūpuna by providing resident advocacy, care coordination, and psychosocial support, ensuring each resident receives culturally sensitive, patient‑centered care.

We are looking for a passionate Social Services Assistant to provide a range of services to community residents and families from the time of admission, through their stay, and through discharge, appropriate to the resident's needs.
Responsibilities- Completes resident social histories, discharge plans, and also develops care plans as part of the inter‑disciplinary team.
- Ensures ongoing psychosocial assessment and assists residents with access to additional services as needed throughout their stay at Hale Makua Health Services.
- Graduate from a recognized college or university with specialized training or degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, psychology, or equivalent combination of education and experience.
- One year of experience in a health care setting working directly with individuals.
- Current and valid 2‑Step TB clearance and health review (can be done in‑house).
