Ohana Pacific Health on Maui is seeking a caring Community Health Worker to support kūpuna with transition of care, health education, and social services in hospital, rehab, and community settings. This is an on-site position with no remote work.
You will collaborate with APP, assist with referrals, enable remote visits when needed, and help patients overcome barriers to care. This role requires empathy, reliable transportation, and a health-related degree or training.
