Position: Unit Care Coordinator (Registered Nurse/RN)

Are you a passionate and dedicated Registered Nurse looking to make a real difference in patient care? As a Unit Care Coordinator , you will play a vital role in overseeing and guiding our nursing staff to deliver exceptional care in a post-acute setting. Your leadership will ensure compliance with all laws, regulations, and facility standards while fostering a supportive environment for both patients and staff.

Position Overview

As the RN Unit Care Coordinator, you will:

  • Supervise and manage patient care through meaningful interactions with patients, families, and nursing staff.
  • Implement and coordinate care plans that align with Life Care standards and regulations.

What You’ll Need

To thrive in this role, you should have:

  • A nursing diploma (associate’s or bachelor’s degree in nursing).
  • A current and active Registered Nurse (RN) license in good standing.
  • At least one year of geriatric nursing experience (preferred).
  • CPR certification upon hire, with the ability to maintain it throughout your employment.

Your Key Responsibilities

In this dynamic role, you will:

  • Lead daily functions of unit nurses and CNAs, providing guidance and support.
  • Chart patient information accurately and in a timely manner.
  • Manage and coordinate patient care to help them achieve their highest potential.
  • Administer medications as prescribed and respond effectively to emergencies.
  • Exhibit outstanding customer service and maintain a positive attitude towards patients.
  • Assist in patient evacuations when necessary.
  • Communicate effectively within an interdisciplinary team.

Working Conditions

As a Unit Care Coordinator, you will:

  • Engage in a variety of physical activities, including sitting, standing, bending, and lifting.
  • Work in a fast-paced environment that requires good judgment and reasoning skills.
