Managed Care Coordinator, Full-Time, Day Shift

Do you have a passion for patient‑centered care and a knack for coaching high‑risk populations? Adventist Health is seeking a full‑time Managed Care Coordinator to collaborate with multidisciplinary teams in our physician office settings.

Works with multidisciplinary healthcare teams in assigned physician office settings. Focuses on coaching and coordination of care for designated patients as well as motivating high‑risk patient populations to complete wellness screenings as required by individual health plans. Works alongside physicians and office staff to identify patient needs and develop processes for managing the patient population. Promotes patient‑centered care and actively participates in multidisciplinary patient‑centered team meetings.

• Assess patient needs and develop care plans in collaboration with medical home/neighborhood physicians.
• Work with insurance companies to enroll patients in case‑management services and serve as liaison between physician offices and population‑health initiatives.
• Develop standing orders and protocols; review population data based on insurance contracts.
• Maintain documentation, track care provided, and report via databases to ensure reimbursement and quality performance.
• Provide patient and caregiver education, distribute preventive screening material, and make outreach calls.
• Collaborate with a team‑based care environment, participate in training or cross‑training, and provide backup coverage.
• Perform any other job‑related duties as assigned.

• High School education/GED or equivalent (Preferred).
• Associate’s/Technical Degree or equivalent combination of education and experience (Preferred).
• Three years of related experience (Preferred).
• Motivational experience (Preferred).
