Role: Licensed Practical Nurse (LPN) providing high-quality, technology-enabled patient care in a dynamic healthcare setting.
Key Duties- Partner with RNs and physicians to deliver hands-on clinical care.
- Administer medications and perform other routine clinical tasks.
- Document patient care accurately in advanced electronic health record systems.
- Support patient visits both in-person and virtually.
- Assist patients in using digital health tools.
- Contribute to workflow improvements and continuous process enhancements.
- Current LPN licensure.
- Strong clinical judgment and patient care skills.
- Proficiency with electronic health records and documentation.
- Ability to collaborate effectively with an interdisciplinary care team.
- Comfort supporting virtual care and digital patient tools.
- Commitment to continuous learning and a strong focus on patient/member experience.
