** Logan Health Hospice is looking for an independent, self-directed RN with strong clinical nursing skills to be part of their compassionate Interdisciplinary Team!  Working for hospice puts you back where nursing started: at the bedside. You'll build real relationships with patients and families, guiding them through symptom management, education, and comfort care at the end of life's journey.
** Here's some of what you'll do:
** You'll manage your own patient caseload and travel throughout the Flathead Valley visiting patients where they call home.
* Be the primary nursing contact for patients, coordinating their care from admission through end of life
* Conduct in-home visits for assessment, symptom management, and comfort-focused interventions
* Educate patients, families, and caregivers on what to expect and how to provide care and symptom management
* Collaborate with our interdisciplinary team — physician, social workers, chaplains, CNAs, LPNs, and volunteers to create a customized patient plan of care
* Attend regularly scheduled Interdisciplinary Team (IDT) meetings
* Provide emotional support and steady presence for families navigating anticipatory grief
* Participate in on-call shift rotation and perform ad-hoc patient visits as needed
* Document visits and coordinate medications, equipment, and supplies to keep care seamless  Must be comfortable driving in winter weather and have a reliable vehicle for transportation.
