As a Licensed Practical Nurse (LPN), you will report to the Director of Care while leading and coordinating care delivery within your designated community.
Your role will involve assessing residents’ needs, implementing individualized care plans, and ensuring the holistic wellness of each resident. Collaboration with physicians, families, and interdisciplinary teams is crucial to enhancing resident care and quality of life.
Key Responsibilities:
• Assess residents’ needs and evaluate care plans
• Coordinate with healthcare professionals for quality care
• Administer medications within BCCNM guidelines
• Document changes in residents’ conditions accurately
• Support new staff orientation and educational initiatives
Requirements:
• Licensed Practical Nurse credential from BCCNM
• Valid Occupational First Aid Level 1 certification
• Previous long-term care facility experience preferred
• Strong English communication skills, written and verbal
• Ability to lift and carry up to 25 lbs
