In this role, you will deliver client-centered care by assessing, planning, and evaluating needs across the care continuum. As a Nursing Coordinator, you'll form and maintain partnerships with health providers and oversee the work of care aides to ensure effective treatment and recovery strategies.
Key Responsibilities:
• Plan and implement personalized care strategies
• Support and supervise care aides in daily tasks
• Perform mental health assessments and crisis interventions
• Document client care accurately and comprehensively
• Ensure adherence to accreditation standards and policies
Requirements:
• Must be actively registered with BCCNM as an RN
• Experience in wound care is essential
• Excellent communication and documentation skills
• Valid Class 5 Driver's License and transportation
• Proven ability to collaborate within multidisciplinary teams
