This position provides comprehensive support services focused on improving student engagement, reducing truancy and chronic absenteeism, supporting dropout prevention and recovery efforts, and strengthening family-school-community partnerships. The Diversion Case Manager will work collaboratively with school administrators, counselors, attendance personnel, families, and community agencies to identify and eliminate barriers that negatively impact student attendance, academic success, and graduation outcomes. The position also coordinates WRAP (Wraparound) services for students and families requiring intensive individualized support.
This role promotes educational equity, restorative practices, trauma-informed care, and social-emotional support to ensure all students have access to meaningful educational opportunities and resources.
Provide individual and group counseling services for students experiencing emotional, behavioral, attendance, social, or family-related challenges.
Develop and implement intervention plans designed to improve attendance, engagement, academic performance, and behavioral outcomes.
Coordinate and provide WRAP (Wraparound) services for students experiencing truancy, chronic absenteeism, school disengagement, behavioral concerns, or other at‑risk factors.
Collaborate with attendance officers, school administrators, school counselors, social workers, and families to identify barriers to attendance and develop individualized intervention plans.
Conduct home visits, family outreach, and school-based meetings to strengthen family engagement and improve student attendance and academic persistence.
Monitor attendance, behavior, and academic data to identify students at risk of chronic absenteeism, truancy, or dropping out of school.
Collaborate with the high school counseling department to support dropout prevention, dropout recovery, and student re-engagement efforts.
Provide case management and wraparound support services for students returning to school after periods of nonattendance, suspension, withdrawal, or alternative placement.
Assist young teen parents experiencing attendance or engagement challenges by connecting them with childcare resources, parenting supports, transportation assistance, healthcare services, academic accommodations, and community-based WRAP services.
Facilitate effective relationships between home, school, and community agencies through coordinated communication, referrals, and resource support.
Connect students and families with community-based services and external agencies that support mental health, housing, food security, healthcare, transportation, and crisis intervention needs.
Conduct parent education activities and workshops that strengthen the home-school partnership and promote student success.
Participate in multidisciplinary teams including Child Study, Threat Assessment, MTSS, School-Based Intervention, Child Find, Eligibility, Attendance Review, and Family Support Teams.
Serve as a student advocate focused on educational access, equity, restorative practices, and trauma-informed support.
Provide crisis intervention, conflict resolution, de-escalation support, and behavioral interventions as needed.
Maintain accurate case records, attendance intervention documentation, referrals, progress monitoring, and required reports in accordance with division guidelines.
Maintain strict confidentiality and professionalism in accordance with FERPA, HIPAA, and division policies and procedures.
Support the implementation of SEL (Social and Emotional Learning), restorative practices, and trauma-informed approaches within schools and student support programs.
