You will provide comprehensive assessments, treatment coordination, and psychosocial support, including individual counseling and group work. Key Responsibilities:

• Conduct comprehensive assessments for designated clients

• Develop and coordinate care plans including treatment services

• Provide direct psychosocial care and counseling

• Advocate for client resources and navigation assistance

• Facilitate education and self-management support for clients

Requirements:

• Registered Social Worker certification in British Columbia

• Valid driver's license for travel requirements

• Experience in Palliative or Oncology settings preferred
