About the Role
Provides andfacilitatesprogram opportunities that are conducive to the individual’s life skill development, community inclusion,and independence. Supports the individual’s vocational goals and promotes skill growth toward gainful employmentifdesired by the individual.
Assists clients with their physical, economic, vocational, recreational, social,emotionaland daily life skills development.
Assists clients to achieve the greatest degree of independence and quality of life possible.

Responsibilities
- Develop, implement, and evaluate individualized support plans in collaboration with clients, families, and interdisciplinary teams.
- Provide person-centered support that promotes independence, life skills development, social inclusion, and community participation.
- Assist with daily living activities, behavior support, medication administration, health and safety needs, and crisis response as required.
- Facilitate recreational, educational, vocational and social activities that support client goals and well-being.
- Monitor client progress,maintainaccuratedocumentation, and contribute to program planning
- Collaborate with families, community agencies, professionals, and other stakeholders to coordinate services and supports; perform related duties as assigned.
- Certificate in a related Human Services, Social Services, Community Support, Disability Support, or equivalent field.
- Minimum one (1) year of recent related experience.
- Equivalent combinations of education, training, and experience may be considered.
- Strong communication, interpersonal, and problem-solving skills.
- A Class 5 Driver’s License is required as a condition of employment.
- Current First Aid/CPR and foodsafecertificationisan asset.
- Licensed homes require a negative TB test result
- Must be legally able to work in Canada
