Schedule: Day shift

Salary Range: $17.50 - $27.25

Good Samaritan – Pohai Nani is a 44-bed, 5-star rated skilled nursing facility specializing in post-acute rehabilitation, long-term/skilled care, and outpatient therapy programs. This role supports residents by planning and delivering meaningful activities and interventions, both one-on-one and in groups, to improve quality of life.

• Administers the monthly activity calendar and ensures activities are organized and upbeat.
• Regularly elicits input from patients/residents to inform activity planning.
• Lead and participate in activities such as music, crafts, and spiritual activities per state requirements.
• Assist with simple nursing procedures and transport residents to and from rooms.
• Assist with documentation in MDS/EPIC systems as required.
• Demonstrate creativity, leadership, and the ability to work under supervision.
• Coordinate with residents with varying needs and behaviors; prioritize and manage resources effectively.

• High school graduate or equivalent preferred.
• Experience in social services or creative activities and geriatrics preferred.
• Knowledge of working with a diverse resident population.
• Preferred current CNA certification or enrollment in a healthcare-related program.
• Basic Life Support (BLS) required based on location; CPR training may be required with six months to obtain certification.
• Ability to obtain and maintain required department-specific competencies and certifications.
• Driver status may be required (Sanford Category I or II) with valid license and ability to drive per policy.
