Administers personal care and assists with activities of daily living for patients in their homes. Reports observations of the patient's physical and psychosocial status to the Registered Nurse (RN), Therapist, or Clinical Supervisor.

Education and Experience:

• High School Education/GED or equivalent:
  Preferred
• Bachelor’s Degree or related experience:
  Preferred
• Experience with home health or community work:
  Preferred

Licenses/Certifications:

• Home Health Aide (HHA) license:
  Required in CA
• Certified Nursing Assistant (CNA) license:
  Preferred in CA
• Basic Life Support (BLS) certification:
  Required
• Valid Driver’s License and age at least 21:
  Required

• Follow treatment plans and assist with personal care tasks such as bathing, hair, skin, mouth, and nail care.
• Assist with mobility, dressing, elimination, and prescribed exercises.
• Perform household tasks essential to patient care and maintain equipment.
• Observe and report changes in the patient's condition.
• Perform other duties as assigned.
