This advanced role is integral to our behavioral health service line, responsible for conducting comprehensive assessments to determine appropriate levels of care for patients, managing referrals, and providing essential support to ensure a seamless entry into care.
The Admission & Referral Specialist II will utilize their clinical expertise to ensure that patients receive timely and effective behavioral health interventions. This position demands a high level of clinical acumen, critical thinking, strong communication skills, and compassion. Additionally, this role requires flexibility with assignments within professional scope/duties/licensure.
- Conduct Comprehensive Assessments:
Perform detailed clinical assessments to evaluate patients' behavioral health needs, use clinical judgment to determine appropriate levels of care, collaborate with physicians, and develop initial treatment plans based on assessment findings. Collect and document relevant patient information, including medical history, presenting problems, and psychosocial factors. - Coordinate Referrals:
Oversee the referral process, ensuring that patients are connected to suitable behavioral health services and specialists within Riverside Healthcare. Manage complex cases, including those with multiple or high-level needs. - Conduct Initial Assessments:
Perform thorough initial intake assessments to determine the behavioral health needs of patients. Collect and document relevant patient information, including medical history, presenting problems, and psychosocial factors. - Provide Clinical Consultation:
Offer expert consultation to patients, families, and healthcare providers regarding behavioral health services, treatment options, and care coordination. Address complex issues and provide clarity on service pathways. - Provide Information and Guidance:
Offer clear and empathetic guidance to patients and their families regarding available behavioral health services, program eligibility, and the next steps in the care process. - Monitor and Follow Up:
Track patient progress and follow up on referrals. Document and report on the effectiveness of services and any issues that arise. - Enhance Service Delivery:
Identify and recommend improvements to intake and referral processes. Implement best practices to streamline workflows and enhance patient satisfaction and care efficiency. - Maintain Accurate Records:
Ensure thorough and accurate documentation in the electronic health record (EHR) system. Analyze data to monitor trends and support continuous quality improvement efforts. - Collaborate with Healthcare Providers:
Work closely with healthcare providers, including physicians and therapists, to support integrated care and facilitate communication regarding patient needs and service coordination. - Address Patient Inquiries:
Respond to patient inquiries promptly, resolve concerns, and provide information about behavioral health services, insurance coverage, and financial assistance options. - Engage in Professional Development:
Participate in ongoing training and professional development to remain current with industry standards and advancements in behavioral health. - Support Departmental Projects:
Contribute to departmental initiatives, quality improvement projects, and team activities as needed to support overall service goals. - Assist with Administrative Duties:
