The Hospital Liaison plays an essential role in ensuring a seamless transition for all admissions from acute or post‑acute care. Your collaborative engagement is significant to coordinate admissions and communication to maintain continuity and provide the best care our communities expect. You will generate and manage referrals and admissions from hospitals, develop relationships with discharge planners, and lead initiatives aligned with market growth.
Key Responsibilities- Stationed daily at the primary account to maximize market engagement and referral review efforts.
- Review and update the CRM dashboard for accurate reporting via Forecast and other communication platforms.
- Respond to all incoming referrals on Careport and other platforms promptly, daily.
- Provide definitive answers for referrals within an hour of receipt, or by end of day if appropriate.
- Communicate with Business Development and Operations leadership to expedite delays, barriers, and solutions in real time.
- Collaborate with AD Concierge on daily updates regarding pending referrals for Forecast.
- Coordinate with Transitional Advisors on communication with incoming referrals and admissions on Care Port and other platforms.
- Enter PCC data timely, including patient demographics, resident representative, POA, and guardian information.
- Prepare weekly reports.
- Participate in quarterly business development meetings to align initiatives that grow census and market exposure.
- Complete daily Forecast for team review via census call, morning meetings, and/or standdown.
- Oversee guest relations and concierge initiatives, including patient rounding, testimonial assistance, and promotion of positive outcomes.
- Minimum: GED or high school diploma with 2 years of experience, or associate degree in healthcare, business or similar with 1 year experience.
- Must meet health assessment requirements regulated by the state.
- Must be capable of performing the essential functions of the job.
