Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in Well Sky Kinnser EMR to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes.

• Provide home health nursing services to patients in Kankakee County.
• Assess patient status and develop care plans.
• Coordinate with interdisciplinary team to prevent hospitalization.
• Document care and respond to clinical emergencies.

• Valid Illinois RN license.
• Valid CPR Certification.
• Valid Driver’s License.
• Valid Auto Insurance.
• Minimum 1 year home health experience.
• Proficiency in Well Sky Kinnser EMR.
• Strong clinical assessment and communication skills.
