CHP is a non‑profit organization that delivers primary health care to migrant farm workers and other medically underserved populations across northern and central Illinois. We specialize in primary preventative medical, dental, and behavioral health services, offering culturally responsive care to patients, caregivers, and community partners.
Location & Hours• Kankakee, IL
• Full‑time
Base pay: $26.00/hr – $31.00/hr (total compensation may vary based on skills and experience)
Key Responsibilities- Deliver high‑quality nursing care to primary care patients, including assessments, treatment, care planning, and education.
- Collaborate with healthcare professionals to develop and implement personalized patient care plans.
- Administer medications and treatments as prescribed by physicians and in accordance with nursing standards.
- Monitor patient progress and adjust care plans based on patient needs and responses.
- Educate and support patients and their families on health management and disease prevention.
- Maintain accurate and timely documentation of patient care, treatments, and progress in electronic medical records.
- Participate in quality improvement initiatives and adhere to regulatory and accreditation standards.
- Maintain confidentiality and protect sensitive patient information at all times.
- Active Registered Nurse (RN) license in Illinois.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 2 years of experience in primary care or a related field.
- Strong clinical assessment and critical‑thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a team‑oriented environment.
- Proficiency in electronic health record (EHR) systems (Athena Practice preferred).
