The Registered Nurse (RN) will provide care for a group of hospice patients, visiting patients in private homes, assisted living facilities, hospitals, and other settings. The RN will complete hospice admissions and patient visits as needed, answer calls from patients, families and customers, and perform nursing visits for symptom management, deaths, and admissions. This role requires passion for hospice care and advocacy for hospice services.
The position operates on an after‑hours on‑call schedule: seven days on/overnight shifts and seven days off.
- Current RN licensure in good standing in the state(s) of practice with at least one year of experience
- Admissions and on‑call experience preferred
- Strong teamwork skills to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- One‑to‑one patient care, supported by a comprehensive patient care team
- Ability to work independently, handling phone, email, written communication, meetings, and hospice regulations
- Excellent time‑management skills for scheduling visits, documentation, and urgent triage
- Familiarity with electronic medical records systems
- Reliable transportation, valid driver’s license, and auto insurance for travel across the service area
