The Clinical Pharmacist provides comprehensive pharmaceutical care for patients, ensuring safe, effective, and cost-conscious drug therapy. Must demonstrate excellent interdisciplinary communication and exhibit strong educational and precepting abilities.

Responsibilities and Essential Job Functions
  • Participate in organizational and departmental performance improvement efforts to enhance pharmaceutical care and department systems.
  • Attend patient care rounds and provide patient-focused pharmaceutical care by assessing and monitoring drug therapy.
  • Support department staffing needs by assisting in coverage and scheduling as needed.
  • Ensure timely medication order processing and review medication orders for resolution of issues.
  • Perform patient-specific medication profile reviews and execute protocol-based drug therapy, adjusting doses for patient-specific parameters.
  • Report medication incidents and adverse reactions, participating in safety reviews.
  • Participate in medication reconciliation and provide patient education.
  • Provide educational presentations and in-services for staff.
  • Precept pharmacy residents, students, and interns, providing mentorship and feedback.
  • Perform professional, clinical, and technical competencies as required.
  • Minimum

    Education and Experience
    • Bachelor's Degree in Pharmacy
    Preferred

    Education and Experience
    • Postgraduate training (PGY-1 and/or PGY-2 residency)
    Required Licensure and Certification
    • ACLS, PALS, NRP, BLS certifications
    • Licensed Pharmacist - State Board of Pharmacy
    Preferred Licensure and Certification
    • Board Certification in area of specialty
