Nurse Practitioner - Midwest; Multi-State Traveler

We are seeking a dedicated Nurse Practitioner to provide high-quality, hands-on care to seniors in post-acute and senior living facilities across the Midwest region. You will support residents during key moments in their health journey, build strong relationships with care teams and facility staff, and grow within an innovative, patient-centered organization that values autonomy and variety.

• Provide direct care for seniors in partner facilities, including skilled nursing, long-term care, assisted living, independent living, and memory care.
• Travel regionally (approximately four days per week) to deliver consistent, high-quality care where it is needed most.
• Conduct assessments, manage chronic and acute conditions, and ensure continuity of care.
• Collaborate with facility leaders, nursing staff, and Curana’s multidisciplinary care team to improve outcomes and address resident needs.
• Serve as an ambassador for Curana Health, fostering strong relationships and supporting smooth transitions to permanent providers.
• Maintain accurate, timely documentation and billing in alignment with policy and compliance standards.

• NP degree with unrestricted licensure (ability to obtain additional state licenses within 30 days).

This role requires travel to and from one or more assigned facilities using a personal vehicle. A valid driver’s license, current auto insurance, and an acceptable driving record are required. Periodic motor vehicle (MVR) checks may be conducted as a condition of, and/or for, continued employment.
