The RN Case Manager, under the supervision of the Director of Continuum of Care Department provides oversight and intervention through assessing, planning, coordinating, monitoring and evaluating services required to respond to individual patient needs.

• Responsible for utilization review, discharge planning and care coordination with patient, patient's family, physician and health care team.
• Works with other members of Continuum of Care Department sharing responsibilities as assigned to meet each patient needs.
• Plan, implement and evaluate patient care plans before, during and after the procedure to optimize outcomes and maximize available resources.

• (RN) Registered Nurse
• Registered Nurse Diploma
• Minimum of one year of acute clinical nursing experience preferred.
• Two years Managed Care and/or Hospital Case Management experience preferred
