AboutThe Role
The Registered Nurse Case Manager plans, organizes, and directs hospice care. The nurse has experience in nursing with an emphasis on community health education. They build on community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.
Job Responsibilities Patient Care- Completes an initial, comprehensive, and ongoing assessment of the patient and family to determine hospice needs, including a full physical assessment and history of current and previous illnesses.
- Provides professional nursing care by utilizing all elements of the nursing process.
- Assesses and evaluates patient status by writing and initiating a plan of care and regularly re‑evaluating patient and family/caregiver needs.
- Participates in revising the plan of care as necessary, initiating appropriate preventive and rehabilitative nursing procedures, and administering medications and treatments as prescribed by the physician.
- Counsels the patient and family in meeting nursing and related needs, provides health‑care instructions, assists with activities of daily living, and facilitates efforts toward self‑sufficiency and optional comfort care.
- Acts as Case Manager when assigned by the Clinical Supervisor and assumes responsibility for coordinating patient care for the assigned caseload.
- Completes, maintains, and submits accurate and relevant clinical notes regarding the patient’s condition and care given, recording pain/symptom management changes/outcomes as appropriate.
- Communicates with the physician regarding the patient’s needs, reports changes in condition, and obtains/receives physician orders as required.
- Communicates with community health‑related persons to coordinate the care plan.
- Teaches the patient and family/caregiver self‑care techniques and provides medication, diet, and other instructions as ordered by the physician, recognizing and utilizing opportunities for health counseling.
- Collaborates with the interdisciplinary group, maintains a safe environment, assists the patient’s family/caregiver and other team members in providing continuity of care, and attends interdisciplinary group meetings.
- Participates in on‑call duties as defined by the on‑call policy.
- Ensures that arrangements for equipment and other necessary items and services are available.
- Supervises ancillary personnel and delegates responsibilities when required.
- Maintains and upgrades professional knowledge, practices skills through continuing education and in‑service classes.
- Fulfills the obligation of requested and/or accepted case assignments.
- Actively participates in quality assessment, performance‑improvement teams, and activities.
- Graduate of an accredited school of nursing.
- Preferred: 1–2 years of recent acute care experience in an institutional setting and 1–2 years of recent experience in home care.
- Current licensure in State and CPR certification.
- Bachelor’s degree, with one year of hospice care experience preferred.
- Management experience not required; responsible for supervising hospice aides.
- Excellent observation, verbal and written communication, problem‑solving, basic math skills, and nursing skills per competency checklist.
- Physical ability to walk or stand for prolonged periods, lift and transfer patients, lift supplies and equipment, and perform reaching, stooping, bending, kneeling, and/or crouching; adequate visual acuity and hearing to perform required nursing skills.
