Sage Group Health Care  Staffing is seeking Registered Nurse Home Health for a 13-week travel/ contract position in NKC, MO. Accepting both local and traveler candidates.

Pay Rate:

Range $1900to $2200 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments

Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri, Must meet 32 productivity points per week

• Registered Nurse: 2years - Required
• Recent/Current Home Health Nursing experience: 1 year – Required
• OASIS experience - Required
• Home Care Home Base(HCHB) charting experience – Highly preferred
• Must be Covid Vaccinated or provide medical or religious exemption

• ASN or an equivalent– Required
• BSN or an equivalent– Highly preferred
• RN License for State of MO or Compact - Required
• BLS Certification from AHA or American Red Cross - Required

• Provide and coordinate care for assigned Home Health patients
• Develop and implement nursing care plans for patients as assigned.
• Empower patients and families to reach healthcare goals by educating them on disease management.
• Provide Home Health nursing care and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
• Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
• Ensure patients’status is assessed on an ongoing basis and pertinent information is gathered and documented.
• Notify physicians of significant changes in status.
• Document and complete charting
