Pay: $38–$40 per hour

• Visit HCBS clients every 60 days to keep them safe, stable, and thriving.
• Provide Delegated Nursing Task (DNT) training to caregivers.
• Monitor wellness & follow up on changes.
• Collaborate with Case Managers to support client success.
• Serve clients on the FE, BI, and PD Waivers.
• Help people stay safely in their own homes where they want to be.

• 40 hours/week
• No on-call
• No weekends
