• Will be working remote when not in the field. 80% field, 20% remote.
• Manages 60-80 patients per caseload.
• Will be given a lap tap, cell phone and office supplies.
• Will be traveling to member homes, long term care facilities, physician offices, hospitals and other provider offices.

• We are looking for field case managers who have experience with geriatric populations.
• Must be willing to travel to patient’s homes.
• Must be computer literate. Big! – Will be asked if they can operate Windows based systems.
• 701B Comprehensive Assessment Certificate is strongly preferred – an exam you take that allows you to conduct 701B type of assessment.
• IT will provide candidates with the skills and knowledge they need to succeed as a case manager or assessor working with elders and adults with disabilities.
