** Position Summary
** As a Palliative Care Nurse Practitioner (NP) with Impact Health PC in
** Katy, TX**, you will provide compassionate, patient-centered care to individuals with serious and complex illnesses within skilled nursing facilities. You will assess symptoms, develop individualized treatment plans, support goals-of-care discussions, and collaborate closely with patients, families, facility staff, physicians, and interdisciplinary care teams.###
** Key Responsibilities

* Conduct comprehensive assessments and develop individualized treatment plans.
* Manage symptoms and provide ongoing follow-up for patients with serious and complex illnesses.

* Collaborate with physicians, facility staff, and interdisciplinary care teams.
* Educate patients, families, and facility staff regarding palliative care and treatment plans.
* Complete timely and accurate clinical documentation.
* Maintain compliance with facility, state, and federal regulations.###

** Qualifications* Active Nurse Practitioner (NP) license in Texas.
* Minimum 2 years of post-certification experience preferred.
* Palliative care, SNF, hospitalist, emergency department, or ICU experience preferred.
* Strong communication and interpersonal skills, particularly when working with patients and families.
* Ability to work independently and collaboratively as part of an interdisciplinary care team.
